In all of that cases I cuoldn't achieve the anaesthysia in the first molar.

did you take the xray(s) before or after you could not achieve anesthesia?

how many years did it take you to get this collection of cases?

how did you eventually achieve anesthesia?

In the upper two the ortopantomographs were taking before treatment as a suspection to impacted molars.
The 3rd one was taken in the emergency care were I take shifts, he had his mandible fractured at the angle and a lot of tooth fractured, some of the needed endo work and it was a failure with the first molar.
The lower 2 are a case of a profesor I work with, he performs implants, and as you can say that was THE CASE.
So this cases I colected since I begun to work in the practice for like 6 years. Belive it or not I think It's a lot of them but many of us just don't pay atention to them.
What I usually do when it fails is the intraligamentary or intrapulpal anaesthisia thou I Know what you are going to say about the intrapulpal.
All of the anaesthesias were performed initially at the mandible foramen, and the numbness of the lip plus tip of the tongue was instaled.

interesting topic. as soon as i 'll learn to upload pics, i'll post some cases (CT scans) where you can find IAN in the interforaminal region.
